The thyroid sets the pace of your metabolism. When it is underactive, everything slows: fatigue, weight gain, feeling cold, constipation, dry skin, low mood, heavy periods. When it is overactive, everything speeds up: weight loss, racing heart, anxiety, sweating, tremor and trouble sleeping. Both are diagnosed with a simple blood test.
At Voss Family Clinic, your provider checks TSH and, when needed, free T4, T3 and thyroid antibodies, examines the gland for enlargement or nodules, and starts treatment. Hypothyroidism is managed here with levothyroxine and regular TSH checks. Hyperthyroidism and nodules are evaluated here and co-managed with endocrinology when needed.

Getting levothyroxine right
Levothyroxine replaces the hormone your thyroid is not making. The dose is adjusted until your TSH is in the target range, usually over a few months, and then checked once or twice a year. Most people feel noticeably better within weeks of reaching the right dose.
Taking levothyroxine correctly
- Same time every day, on an empty stomach, with water
- Wait 30 to 60 minutes before eating or coffee
- Separate from calcium, iron, antacids and some supplements by 4 hours
- Do not switch brands or generics without telling your provider
- Tell us if you become pregnant: doses usually need to rise

TSH is the main screening test. Free T4, T3 and thyroid antibodies are added when the TSH is abnormal or symptoms suggest a specific cause.
How often will my thyroid be checked?
Every 6 to 12 weeks after a dose change, then every 6 to 12 months once your TSH is stable.
Can thyroid problems cause weight gain?
An underactive thyroid can cause modest weight gain and makes losing weight harder. Treating it helps, but weight management usually needs its own plan too.
Do I need an endocrinologist?
Hypothyroidism is usually managed in primary care. Hyperthyroidism, nodules, thyroid disease in pregnancy and complex cases are co-managed with endocrinology.
Is a mildly high TSH always treated?
Not always. A mildly elevated TSH with normal free T4 (subclinical hypothyroidism) is repeated and treated based on symptoms, antibodies, age and pregnancy plans.
